“A Chemical Dependency Case Manager was unable to work, but had moved to Texas because the warm weather helped her COPD and joint pain. While she was working on establishing relationships with her new treating providers in Texas, The Hartford terminated her disability benefits.
Fields Disability assisted by appealing the determination on her behalf. Our attorneys ordered updated medical records and requested support from our client’s new doctors. The updated information was submitted to The Hartford, and, after a brief review, her benefits were successfully reinstated.”

“A Pharmacy Technician from Texas was removed from all work activities due to a work-related injury to the right knee. She applied for Long Term Disability benefits under her employer’s policy. Her claim was initially approved, but then Prudential denied her ongoing benefits stating that the medical records did not support continued total disability. She produced the additional medical information requested by Prudential, but was told that the information submitted still did not support her claimed restrictions and limitations.
Upset that she was unable to get Prudential to change their decision on her claim, she decided to contact the attorneys at Fields Disability. Our disability team requested all of our client’s medical records, functional and vocational analysis reports. After organizing and reviewing the supporting information, the Fields Disability attorneys drafted an appeal stating the faults in Prudential’s analysis of our client’s claim. After reviewing the information received on appeal, Prudential reversed the denial. Our client was paid back benefits for all of the months that were outstanding, and she continued to receive benefits.”

“A Senior Service Technician at a power plant in Texas had to stop working due to chronic pain and failed back syndrome. After two years of benefits, Prudential terminated his Long Term Disability, stating that at this point nothing would prevent him being able to work a sedentary job. Upon receiving this news, he decided he needed the assistance of the attorneys at Fields Disability to submit an appeal.
Our disability team acquired medical records, functional capacity evaluations, and a vocational abilities report. When all the supporting documentation was gathered, the Fields Disability attorneys submitted an appeal. On review, Prudential reversed the decision to deny our client’s Long Term Disability benefits. Our client received a check for all past due Long Term Disability benefits, and he continued to receive his disability benefits in the years following the decision.”

He called Fields Disability for help. Our team obtained the claim file from the insurer and found that they had made several errors when the medical evidence was reviewed. The team gathered updated documentation supporting the claimed level of impairment. Our attorneys drafted a strong appeal to MetLife based on the errors that were identified and the updated medical records. MetLife reversed the decision on appeal, and our client received a lump-sum payment for the back benefit period, and ongoing monthly benefits.”

Liberty Mutual - Grundy, VA

“A Safety Manager from Virginia suffered from degenerative joint disease in his knee, a compression fracture to the thoracic spine requiring fusion surgery, spondylolysis, and arthritis. His conditions progressed until he was unable to work. He applied for benefits under his disability policy and received two years of Long Term Disability benefits. One month before the definition of disability changed, Liberty Mutual determined his medical records did not support total disability under the any occupation definition of disability and closed his claim.
He reached out to Fields Disability, and our attorneys developed a plan to gather the information necessary to win his appeal. Our disability team requested updated narratives from several of his treating physicians. The Fields attorneys detailed how Liberty Mutual had failed to properly credit our client’s favorable Social Security Disability award, and we also asserted that many of the occupations identified by a vocational consultant relied upon by Liberty Mutual to discontinue benefits were actually outside of our client’s limitations and restrictions. On appeal, Liberty Mutual reversed the denial decision. Our client received a check for his missed benefits, and he started receiving monthly benefit payments again.”
