Warehouse Associate - Memphis, TX

Vertigo, Fibromyalgia

“A warehouse associate from Tennessee became disabled when she developed vertigo and fibromyalgia. She received Short Term Disability benefits for a few months, until Cigna denied her continuing benefits stating her medical records did show that she was disabled.

Unable to work and worried about the loss of income, she hired Fields Disability to appeal the denial. Our disability team collected medical records and medical information from her doctors in support for her claimed disability. The Fields attorneys submitted additional medical information for review and argued in support of our client’s claim. Cigna reversed the denial and paid our client all her past due benefits, reinstating her disability claim.”

Purchasing Agent - Rhome, TX

Chronic Back Pain, Ischemic Heart Disease

“A Purchasing Agent for a steel company in Texas became unable to work once her debilitating pain caused by sacroiliitis, degenerative disc disease, and ischemic heart disease became too severe. Benefits were paid for two years, then The Hartford denied the claim stating that the records did not support total disability anymore.

Unable to work and not knowing what she was supposed to do, she sought the help of Fields Disability. Our team requested additional information from her treatment providers and obtained a report from our client’s physician clearly outlining her conditions. The Fields attorneys reviewed the records produced by The Hartford, and the updated records. We also sent her for testing, which objectively demonstrated she did not have the ability to perform any type of work. All the materials were submitted to the insurer for review on appeal. After review, the denial was reversed, our client was paid all past due benefits, and The Hartford reinstated the payment of monthly benefits. ”

Law Office Manager - Friendswood, TX

Chronic Pain, Insomnia, Depression

“A law office manager in Texas was no longer able to work because her chronic back pain was getting so severe that she couldn’t remain seated or sleep properly. She applied for disability benefits under her policy. She was approved for Long Term Disability benefits, and received monthly benefits. After two years, Prudential abruptly ended her benefits stating she no longer met the new policy definition of totally disabled.

Worried about her lost income, she contacted Fields Disability for assistance. The team contacted our client’s treating physicians, and obtained updated medical reports. The Fields attorneys scoured the denial documents noting that Prudential ignored recommendations from one of her treating physicians. After researching the medical record and the file, we submitted a strong appeal. Prudential reversed their denial, and our client received all past due benefits.”

Chemical Dependrncy Case Manager - Marcus, TX

COPD, Chronic Pain

“A Chemical Dependency Case Manager was unable to work, but had moved to Texas because the warm weather helped her COPD and joint pain. While she was working on establishing relationships with her new treating providers in Texas, The Hartford terminated her disability benefits.

Fields Disability assisted by appealing the determination on her behalf. Our attorneys ordered updated medical records and requested support from our client’s new doctors. The updated information was submitted to The Hartford, and, after a brief review, her benefits were successfully reinstated.”

Pharmacy Technician - Humble, TX

Degenerative Disc Disease, Right Knee Chondromalacia, Status Post Right Lateral Meniscus Repair and Total Knee Arthroplasty, Memory Loss, Dizziness, Shortness of Breath, Unstable Gait, Diabetes Mellitus, Diabetic Ketoacidosis Episodes, Acute Renal Failure, Hypertension, Hyperlipidemia, Anemia

“A Pharmacy Technician from Texas was removed from all work activities due to a work-related injury to the right knee. She applied for Long Term Disability benefits under her employer’s policy. Her claim was initially approved, but then Prudential denied her ongoing benefits stating that the medical records did not support continued total disability. She produced the additional medical information requested by Prudential, but was told that the information submitted still did not support her claimed restrictions and limitations.

Upset that she was unable to get Prudential to change their decision on her claim, she decided to contact the attorneys at Fields Disability. Our disability team requested all of our client’s medical records, functional and vocational analysis reports. After organizing and reviewing the supporting information, the Fields Disability attorneys drafted an appeal stating the faults in Prudential’s analysis of our client’s claim. After reviewing the information received on appeal, Prudential reversed the denial. Our client was paid back benefits for all of the months that were outstanding, and she continued to receive benefits.”
